Checklist
Application for Enrollment in Grades TK-8
Submit an application for the 2024-2025 academic year here.
Include the following materials with your application:
- Copy of your child’s Birth Certificate
- Copy of child’s Baptismal Certificate (Catholic Student Applicants only. Catholic families who are not active within the St. Theresa Parish must provide us with a letter from a current Pastor/Parish confirming active parishioner status.)
- $85.00 non-refundable application fee (paid online through the above link)
- Copy of your child’s most recent two Report Cards (if applicable) or letter of progress from pre-school. Note: If applying for grades two through eight, upload previous year end and current year report cards and standardized test scores. If applying for first grade, please upload report card for current year.
- School Evaluation Form from current teacher or principal.
A school recommendation form is required for all applicants. We do understand OUSD’s recommendation letter policy and we invite you to provide the form to any tutor or other academic resource, if able. This form should be given to the teacher and sent directly back to St. Theresa School via USPS or by email directly to admissions@sttheresaschool.org

Once your application is submitted, we will contact you to confirm receipt of all required forms. Soon after, an academic assessment will be scheduled. In the meantime, please register for a FACTS account by visiting this link. You may also consider beginning to gather all recent immunization records and verification of a recent dental visit. These documents are collected in the Spring, shortly after acceptance.
Academic Assessment
All applicants must be scheduled for an academic assessment, so please save the date:
- TK/Kindergarten: various times beginning at 2:30pm on Wednesday, February 12, 2025 (by appointment only)
- Grades 1-8: Wednesday, February 26, 2025 at 3:00pm
- TESTING WILL NOT BE SCHEDULED UNTIL ALL PAPERWORK IS RECEIVED
Academic assessments look different dependent on grade level. This is a wonderful opportunity for students to visit our classrooms, meet members of our teaching staff, and mingle with other applicants. In younger grades, students play with one another and are led through age-appropriate activities that help our staff to assess their academic and developmental readiness. In older grade levels, students are expected to complete an abridged STAR test and provide a writing sample.
